WHO WE ARE
Harbor Heights of Sheboygan is a supportive care facility for older adults who can no longer live independently. We are locally owned; we are not owned by a corporation or a large chain of facilities...we live, support, and promote this community...and we know the needs and desires of the people who live here. Our mission is to provide the best quality care to our residents who need some assistance to function to their maximum level. The spiritual, physical and social needs of our residents are ever important to us as we strive to make their home with us as warm, comfortable and inviting as we can.

MONTHLY FEES
The rate structure at Harbor Heights is based on the individual needs of each resident, the service or services we provide to them, and the size suite the resident has chosen to live in. This, we feel, is the most equitable way to determine fair costs for each person. We also recognize that needs change as our residents age. The staff responsible for cares and services carefully document the services they provide to each resident, as well as any changes of condition that might take place. This documentation assists us in determining periodic rate changes as each resident's needs dictate.

Harbor Heights is pleased to be able to offer two payment options for you to select from. One is an all-inclusive rate, determined prior to occupancy after our professional assessment is done. This rate will include any services the resident may need at a low, moderate, or high level.

Rates begin at: $2355 / month

The other payment option allows you to pay a base rate, which includes your choice of suite, weekly housekeeping, laundering of linens, three home-cooked meals daily, self-medications support, activity programming, all utilities (except phone), and our 24-hour awake staff. We determine if the resident needs any supportive services during the first 30 days of residency after staff has had an opportunity to work with and to get to know the specific needs of the individual.

Those supportive services are discussed, and if agreed upon with the resident, they are then charged on a monthly basis.

* Rates are subject to change and we will always give a 30-day notice prior to any rate changes.

The enclosed application must be completed on both sides and returned to the Business Office for review and approval. A deposit of $500 will be required upon acceptance, which will become the Security Deposit, refundable at the time of move-out subject to carpet cleaning charges or other damages.
